    Understanding Headache Types

    The American Headache Society, of which Dr. Shirazi is a member, classifies headaches into three primary groups.

    Primary Headaches

    These account for the majority of headache cases and include:

    • Tension-Type Headaches
    • Migraine Headaches
    • Cluster Headaches

    Secondary Headaches

    These make up less than 10% of headaches and result from conditions such as:

    • Brain hemorrhage
    • Tumors (abnormal tissue growth in the brain)
    • Meningitis or encephalitis (inflammation of the brain’s surrounding tissues, leading to increased cranial pressure)

    These types of headaches will not be covered further in this discussion.

    Facial Pain, Cranial Neuralgias and Migraines

    Chronic facial pain and issues related to cranial nerves can cause severe discomfort and are often mistaken for headaches. These conditions may stem from TMJ disorders or nerve compression, leading to persistent pain.

    Headaches can manifest in various forms, and identifying the specific type you’re experiencing is essential for effective treatment. Our focus is on addressing headaches linked to tension, migraines, TMJ disorders, and related conditions.

     

    TMJ Headache Types

    Tension Headaches

    Tension-type headaches are the most common and typically feel like a dull, band-like pressure across the forehead or at the base of the skull. They are often linked to muscle tension, jaw clenching, or poor posture, which can contribute to neck and upper back discomfort.

    Forward Head Posture (FHP)

    Forward Head Posture (FHP), commonly associated with jaw and airway issues, increases strain on the neck and shoulders, leading to chronic headaches. For every inch the head shifts forward, the neck must support an extra 10 pounds of weight, creating significant stress.

    Migraines

    Migraines are intense, often one-sided headaches that can severely impact daily life. These headaches stem from changes in blood vessels, often triggered by the trigeminal nerve, which has connections to the jaw and facial structures.

    Cluster Headaches

    Cluster headaches are severe, recurring episodes of pain, typically concentrated around one eye or one side of the head. Though challenging to manage, professional treatment can help alleviate symptoms and improve quality of life.

    Cranial Neuralgias, Facial Pain and Other Headaches

    The American Headache Society, of which Dr. Shirazi is a member, categorizes headaches into three primary groups.  

    Cranial Neuralgia

    Cranial neuralgia is characterized by recurring pain in specific areas of the head, face, or scalp, with varying levels of intensity. The pain may subside temporarily but often returns, typically following the path of the affected cranial nerve. Depending on the type of neuralgia, the pain can be described as sharp, burning, excruciating, or shock-like, sometimes triggered by even the lightest touch. Some individuals may also experience numbness, itching, or muscle weakness.

    Causes

    Neuralgia can result from multiple factors, with the most common being compression, irritation, or misalignment affecting cranial nerves or upper cervical roots due to structural imbalances. Other potential causes include herpes infections, diabetic neuropathy, Tolosa-Hunt Syndrome (a painful ophthalmoplegia caused by inflammation in the cavernous sinus or superior orbital fissure), or a central nervous system origin, meaning the pain could be “referred” or “sensitized” from TMJ dysfunction.
